New patients are welcome, and the forms can be filled out before you arrive rather than at the desk. Ask for them when you call and we will get them to you.
Bring these four things
- Your insurance information
- Any recent dental X-rays
- Your driver's license or photo ID
- A list of the medications you take
Most insurance is accepted and billed electronically from this office, so the claim goes in from here. For larger treatment we are a CareCredit provider, with no-interest and extended plans for approved patients.
